LTE: How operator will charge user when data will be reaching to UE through LTE radio access as well as through Wifi AP?

Now and days, various technology options are coming out which can be used to improve LTE throughput as well as LTE capacity. LTE-U/LAA, LWA are well known options. Would operators prefer to have different charging fro different access or the user will be charged same whether it is receiving data through any access (i.e. LTE-U/LAA or LWA) ?

LTE Basics: Is only Radio Interface the one which is non-wired rest everything is connected through wire?

As far as I know, radio Interface (AIR INTERFACE) is only between UE and ENODEB rest everything is connected through wire, Is my understanding correct?

If No, then please explain how other things are connected and how data is being transferred?
If Yes.. Then how Long Distance call been made?

LTE: Is same MCS used for all the MIMO layers according to 3GPP standard in case of Tx Diversity and Spacial Diversity?

It seems likely but technically the radio link channel condition of the layers would be different as it is the premise of the MIMO technology..
