LTE: What all information are collected in charging data record (CDR) for an UE ?

When we talk about CDR (charging data record) in LTE or any other technology, following questions come in my mind.
- Is CDR generated per UE ? or Is per session per UE or generated for group of UEs ?
- What information is captured in CDR ? Does 3GPP define any standard format and IEs that need to be captured or is it completely operator dependent ?

Suppose a given UE having some certain IMSI trying to make an Audio call. For that IMSI,apn is defined in HSS and policy for that apn is defined in PCRF .For this apn two bearer will be formed,one default bearer which is used for sip signalling and other dedicated bearer which is used for the actual RTP packets flow.So there will be two charging id.Both bearer will be charged differently and two separate CDR will be generated,one for default bearer and one for dedicated bearer at the end of the call

CDR contains a lot of information,I am just highlighting the main attributes which CDR contains.

Duration of the call.
reason for closing the CDR.
Data consumed during the call(both uplink and downlink)

Please refer below specs for all the mandatory avp and operator conditional avp which is needed in CDR.

Thanks to share your knowledge. But I am surprise to know that default bearer for IMS (with the qci 5 ) is also charged. I thought only the qci "1" bearer will be charged for uplink and downlink data.
CDR is generated for Default Bearer and it is sent to Billing domain with both uplink and downlink data consumed octet with its own service id and rating group.

I am going through the 32.299 and trying to figure out the contents of the CCR message.

Section 6.4.2 lists the AVPS in that message, I am looking for how the PCEF sends the "used bytes" to OCS. The "Multiple services credit control" looks like the one. But need some inputs and help on this AVP. Or please correct me if I am wrong.

What is the difference between Gx and Gz interface.
As we know the interface between P-GW and OFCS is Gz interface and P-GW to PCRF is Gx.
Why both the interfaces are considered as Offline Charging
