Line 2&3 ----> 4 is correct and well placed & 5,6,7 excluded => last digit=4
Line 4&5 ----> 1 is correct but wrongly placed & 0,2 excluded => second digit=1
Line 1 ----> 3 is correct but wrongly placed => first digit=3 answer Mar 28

From the first and third row statements, it is clear that the third digit of the CODE is 4 (since numbers 5 & 6 are invalid) and that the placement of the digits, in the answer would be 3 & 2.
The fourth-row statement clears that the third digit could be 0 or 1. Analysing the first, fourth and fifth row statements, it can be concluded that 314 is the CODE.
